Radial ESWT devices have a pistol-like handpiece. There is a ballistic projectile (or bullet) to create a pressure wave. The bullet hits the tip of the applicator at high speed which creates a pressure wave that can penetrate tissue. The bullet, and the collision generates kinetic energy.

Radial Shock Wave Therapy causes mechanical stress and microtrauma, and this trauma causes the formation of angiogenic factors.
